*Schedule:*
* All shifts & statuses available

*Client Specifics:*
* Client is 14 years old with a vent/trach.

*Requirements:*
* Must have an active RN or LPN license in Minnesota.

*Pay:*
* Registered Nurse (RN): $32.00 - $40.25 per hour (DOE)
* Licensed Practical Nurse (LPN): $25.00 - 31.00 per hour (DOE)

*Core Responsibilities:*
* Provides home care nursing in accordance with the physician's plan of care.
* Reports significant changes in the client's condition to the family, RN Clinical Manager, & physician.
* Obtains physician orders for changing client care needs.
* Maintains accurate & current plan of care, physician's orders, and medication profile for the client.
* Charts cares thoroughly & accurately during shift.
* Provides advice & support to clients & their families.
