Assists Clinic Outpatient and Access Services staff in engaging clients in treatment services, screening and following up with high-risk clients, assisting with care coordination and life skills as needed during transition to other services, and connecting clients to necessary resources. Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model. Performs all required job duties and responsibilities in accordance to CCBHC Standards. Requires effective communication via written, verbal, in person and virtual methods. Knowledge of community geographical awareness and of community and agency resources. Skill in customer service techniques. Education level required is an Associate degree, with a Bachelor’s degree preferred. At least two years of direct service experience working or volunteering in a human services organization is preferred. Physical requirements include standing 20% of the time, sitting 80%, and occasional squatting, driving, kneeling, lifting, and bending. Centerstone is a nonprofit health system providing mental health and substance use disorder treatments nationally through outpatient clinics, residential programs, telehealth, and inpatient hospital services.
